We would like to welcome you to the Specialty Surgery Center. In order to protect your time we encourage you to complete the on-line preregistration process in this section. This will facilitate your admission process on the day of surgery.


Pre-Registration Process

  1. Please review the information on the following page regarding Patient Rights and Responsibilities.
  2. At the bottom of that page will be a link to review the Advance Directives.
  3. At the bottom of the Advance Directives will be a link to complete the Online Registration.  Please follow the guide to fill out your Patient Registration and Health History Information.
